Good day! Incoming Grade 7 students of A.Y. 2020-2021 from public and private schools may now apply for Education Service Contracting Program (ESC) starting July 6, 2020. Deadline of submission is until September 30, 2020. Please note that this is open for enrolled students only. Kindly follow the procedure listed below.

ESC APPLICATION PROCEDURE

Step 1: Click the link provided below. Download the application form and the ESC Grantee Enrolment Contract. Kindly print the form and fill out all the information needed.

Step 2: Please attach all the requirements listed below.

  • 2pcs Recent identical 2×2 ID photo of the student
  • 1 Photocopy of PSA certified birth certificate
  • 1 Photocopy of Grade 6 report card
  • 1 Photocopy of Latest Income Tax Return for the previous year or Original Affidavit of Tax Exemption of Non-Filing of Tax of both parents or legal guardian (incase that ITR is not available).

Step 3: Submit the application form with the complete requirements to the Principal’s Office. Incomplete requirements will NOT BE PROCESSED. All applications received will undergo screening and evaluation process.
