  • Certificate of Good Moral Character
  • Form 138
  • Baptismal Certificate
  • Confirmation Certificate (Kumpil)
  • 1 pc 2×2 photo (white background)

 Transferee:

  • PSA Birth Certificate
  • Certificate of Good Moral Character
  • Transcript of Records
  • Honorable Dismissal
  • Baptismal Certificate
  • Confirmation Certificate (Kumpil)
  • 1 pc 2×2 photo (white background)

*Click the logo of the College in which you are going to enroll.

(For the Teacher Certificate Program and Bachelor in Human Services click on the College of Arts, Sciences and Education icon)
